आमिर खान के टीवी शो ‘सत्यमेव जयते’ से महंगी दवाओं के विकल्प के रूप में सुर्खियों में आईं जेनेरिक दवाओं ने फिर से महंगी दवाओं के विकल्प के रूप कई प्रश्न खड़े किए हैं। इसी बजट सत्र में प्रधानमंत्री ने भी वादा किया था कि अक्टूबर तक सस्ती जेनेरिक दवाएं सरकारी केंद्रों पर मुफ्त उपलब्ध कराई जाएंगी। जेनेरिक दवाओं पर चर्चा शुरु होने के बाद से इस योजना पर भी अमल शुरू हो गया है।
ऐसे में जेनेरिक दवाएं क्या हैं, इनकी कीमत और जरूरत की जानकारी और इनसे जुड़े सभी पहलुओं पर गौर करना जरूरी है। ‘द हेरिटेज’ पत्रिका के संपादक और राष्ट्रीय पुरस्कार विजेता डॉ.एके अरुण से बातचीत के आधार पर हम जेनेरिक दवाओं के बारे में अपनी जानकारी आपसे शेयर कर रहे हैं।
क्या हैं जेनेरिक दवाएं?
जेनेरिक दवा या ‘इंटरनेशनल नॉन प्रॉपराइटी नेम मेडिसीन’ उनको कहते हैं जिनकी कंपोजिशन ओरिजिनल दवाओं के समान होती है। साथ ही ये दवाएं विश्व स्वास्थ्य संगठन की ‘एसेंशियल ड्रग’ लिस्ट के मानदंडों के अनुरूप होती हैं।
इन दवाओं के सारे तत्व यानी इनकी कंपोजिशन, इनकी उपयोगिता और इनका फायदा- सब कुछ ब्रांडेड दवाओं के समान ही होता है, लेकिन इनकी कीमत ब्रांडेड दवाओं की अपेक्षा बहुत कम होती है।
कई बार किसी महामारी या बड़ी बीमारी, जिसमें दवाओं की मांग अधिक हो, उस मामले में ब्रांडेड कंपनियों की दवाओं का लाइसेंस अन्य कंपनियों को भी दिया जाता है जो उन दवाओं की जेनेरिक दवाएं बनाती हैं। या फिर दवाओं का पेटेंट खत्म होने पर भी अन्य कंपनियां मैलिक दवाओं के कंपोजिशन को इस्तेमान कर जेनेरिक दवाएं बनाती हैं।
क्यों कम होती है कीमतें?
आप भी सोच रहे होंगे कि अगर जेनेरिक दवाएं इतनी ही लाभकारी हैं, तो ये बाजार में इतनी सस्ती क्यों हैं? सबसे पहले तो यह जानना जरूरी है कि जेनेरिक दवाओं का अस्तित्व ही इनकी आवश्यकता के स्तर से जुड़ा है। इस वजह से इनकी कीमतों का कम होना वाजिब है।
जिस लाइसेंस के तहत विश्व भर में ये दवाएं बनती हैं, उसमें इनकी कीमतों पर नियंत्रण रखने का प्रावधान है। चूंकि ये दवाएं केवल फॉर्मूलेशन के आधार पर बनाई जाती हैं इसलिए इनकी कीमतों को बहुत अधिक रखा भी नहीं जा सकता।
किसी भी ब्रांडेड या मौलिक दवा को बनाने में कई सालों की रिसर्च, ऊर्जा और धन लगता है। तब जाकर किसी कंपनी को उसका पेटेंट मिलता है और वह कंपनी उस दवा से मुनाफा कमाने के लिए उसकी कीमतों में बढ़ोत्तरी करती है। वहीं, जेनेरिक दवाओं पर केवल इनके निर्माण का ही खर्च होता है, इसलिए इनकी कीमत अपेक्षाकृत कम होती है।
जेनेरिक पर जानकारी कम
जेनेरिक दवाओं के प्रसार में बहुत बड़ी चुनौती है जानकारी और जागरुकता का अभाव। सन् 1990 में जब जेनेरिक दवाओं को खुला बाजार मिला तो कई बड़ी ब्रांडेड कंपनियों ने पेटेंट और कॉपीराइट जैसे मुद्दों को उठाया और इन दवाओं की पहुंच को रोकने के कई प्रयास किए। चूंकि सामान्यतः ब्रांडेड दवाओं से कुछ डॉक्टरों के कमीशन और कंपनियों के हित सीधे जुड़े होते हैं इसलिए रीटेलर के पास मौजूद होने के बावजूद भी डॉक्टरों ने इन्हें प्रिस्क्राइब करने पर जोर दिया नहीं और धीरे-धीरे इन दवाओं को रीटेलरों ने रखना कम कर दिया।
